Purpose

The purpose of this form is to obtain your permission to:

1.collect, use and/or disclose your video recording, voice recording, photograph and/or information provided ("Information") for the purposes of purposes of creating, developing, improving and use of a learning and assessment application, it artificial intelligence technology for education, research and/or quality assurance; and

2.reproduce your Information in reports, teaching sessions, academic and scientific meetings / conferences, and medical / scientific journals as may be required or in connection with and for purposes of the project relating to it and involving CS College (CSC")("Purpose").

Participation

Please understand that your participation is voluntary and that you have the right to withdraw your consent or discontinue participation at any time with no penalty. We will give you a copy of this consent form at your request.

Acknowledgement

By agreeing to this terms and conditions and using Empathy AI, you agree that:

1.you have read this entire form and the Purpose has been explained to you in terms which you have understood;

2.your Information may be collected, used, disclosed and/or adapted by CSC and its collaborators​ for the Purpose;

3.all or part of the Information may be used in conjunction with other video recordings, voice recordings, photographs and information, and complete anonymity cannot be guaranteed;

4.once the Information is made available for research or teaching purposes (which shall include publication), recovery of the Information may not be possible;

5. No fee is payable to you for use of the Information either now, or at any time in the future; and

6. your consent granted herein does not supersede or replace any other consents which you may have previously provided to CSC in respect of your personal data, and is additional to any rights which CSC may have at law to collect, use or disclose your personal data.
